Patient and Family Caregiver Partnership Program

Ventura County Health Care Agency – Ambulatory Care just launched a Patient and Family Caregiver Partnership Program, this new initiative is all about bringing patient and caregiver voices into how to shape patient care, and they are looking to connect with folks in the community who might be interested in getting involved. The goal is to create more responsive, inclusive, and patient-centered care within our ambulatory care settings.  You can make a difference by sharing your insights!
